Carlton Clinic Joint Care Classes at The Empire Hall,
Victoria Road, Horley, Surrey
Rationale
There is evidence that regular exercise can increase an individual's
tolerance to pain in many cases and thus it can be a useful tool
in the management of long term pain sufferance.
Increasing strength, endurance, supplety, and local circulation
to the joints almost certainly not only decreases the likelihood
of injury but may aid a speedier recovery if injury does occur.
The Carlton Clinic Osteopaths have designed a unique system
to obtain for its participants all the toning, stamina, supplety, improved
posture and relaxation.
The Carlton Clinic Joint
Care Classes are ideally suited to those who
wish to achieve these goals but who cannot or do not want to take the
risk factors of other systems. Not only are they highly designed but
tutored at the time also, each class has two tutors personally instructing
the class (not exercising themselves); usually a personal fitness trainer
and a Registered Osteopath.

Joint Care Classes achieve all the
increased supplety of yoga without
using gravity to stretch tissues, the toning of pump/ conditioning/
pilates type classes, the stamina of cardio vascular exercise, along
with improved posture, breathing, and relaxation; all in a safer system
and more controlled environment than any other system.
The principles on which the JOINT CARE exercises are based are:
- Range of movement
- Endurance (stamina)
a) local muscle endurance b) circulo-respiratory endurance (heart
and lung efficiency) c) environmental endurance
- Strength
- Skill
Most activities require a blend of these components and indeed, the
components themselves merge, (strength can be a factor in localised
muscle endurance for instance).
Ranges of movement
You must ensure that there is enough mobility for the activity to be
undertaken. In the case of the back this means spinal, hip, knee, ankle
and toe joint movement in particular. You need muscle flexibility as
well to accompany the joint movements.
The blend of two components is illustrated here - strength is required
throughout the range of mobility necessary to carry out the activity
demanded for strain to be avoided. Strain means injury, and injury can
lead to back pain.
Endurance
a) Local muscle endurance
Deficiency in this type of endurance is a very common factor in causing
strains. Strains account for a large quantity of the spinal problems
that occur both in the lower back and the neck. Repetitive work - for
example, the manual handling of patients on a ward, or the use of a
computer throughout a work shift - can, and often does, result in fatigue
in the joint support muscles. In the case of manual handling, this happens
in the lower back and in computer use, in the neck. This fatigue can
result in ligament strain.
b) Circulo-respiratory endurance, heart & lung efficiency
This is a measure of general fitness. If this factor is poor - the
stamina can produce a total breakdown in adaptive processes. Most people's
fitness is poor- the stamina of the average person will not last through
a work shift. That average person's coping mechanisms will fail and
illness of some type of other will occur. It could well result in joint
pain.
c) Environmental endurance
This means the ability to deal with stress and the various challenges
of life. It covers the ability of people to adapt to specific work situations,
eg housework, keyboard work, nursing other people, gardening or DIY
activities etc.
Stress
Tolerating discomfort and learning to judge how tired you are and
how to cope with these events are all part of this type of endurance.
Motivation for the activity being carried out is another consideration,
as is the ability to compartmentalise work, social and domestic situations.
The spillage of problems from one area of activity into another will
result in loss of efficiency and possible illness.
Many research studies exist demonstrating reduction in stress levels
with general exercise. Good general fitness results from an adequate
balance between physical and relaxation training and the various challenges
of life, JOINTCARE classes can provide this.
Strength
To perform manual handling or any task involving muscle and joint activities,
you need to be strong enough for that task. If continual demands exceed
a person's capacity then musculo-skeletal breakdown will occur, hence
the need for strength training. The response to such training is to
develop a strength reserve above the activity demand.
Skill
Considerable national effort over some years now has been put into
training people in the "skill" of an activity, ie the correct
method of manual handling, the nurse lifting a patient, correct gardening
techniques, moving a carpet roll, etc., are all examples but it has
been found that this type of instructional training is limited in its
effect. Practising skills in the work situation has not proven to be
universally successful either. The factors lacking are good treatment
and physical training, as well as satisfactory ongoing training of various
types including exercises in the workplace. JOINT CARE classes will provide
the trained exercise and access to professionals who can provide the
appropriate treatment.

To make the Joint Care Classes even more personalised to your individual
requirements your current joint mobility, including the spine, muscle
flexibility and circulo-respiratory endurance, can be assessed in advance
of attending the classes by a Carlton Clinic Osteopath to check all
aspects of your suitability to the classes if you wish. This is bookable
at the clinic when you book your classes.
Summary
Osteopaths have been giving the type of exercises that have spawned
modern copies with household names since the 1800's, and the Carlton
Clinic Joint Care Classes are an update on these to suit particular patient
needs.
The JOINT CARE CLASSES main objective is to improve the mobility and
strength of the supporting muscles of the joints. It is important to
appreciate that an episode of pain arising from a joint lesion will
result in some degree of muscle wasting in the supporting muscles. The
resulting weakness of these muscles maybe a possible factor in the recurrent
nature of some joint injuries. To rehabilitate these supporting muscles,
an exercise programme is recommended. It can take up to two years to
achieve adequate muscle support through exercise.
In performing exercises designed for joint care, it is important to
appreciate the value of exercise in relation to improvement of circulo-respiratory
endurance, that being general fitness. Improvement in this endurance
leads to a reduction in blood pressure and resting heart rate and is
vital in the prevention of strokes and heart attacks.
JOINT CARE CLASS members are also given dietary advice to aid weight
loss where appropriate and safe firming and toning exercises where desired.
JOINT CARE CLASSES combine all the advice factors, through its weekly
classes and ongoing monitoring of strength, fitness, mobility and blood
pressure etc, along with individually prescribed Home Exercise Programmes.

your muscles but also ensure a happier and healthier lifestyle.

A common pattern of attendance at the classes would be to complete
at least 20 classes. Then when one is becoming truly familiar with performing
each one properly, start to perform them at home, with attendance back
at the classes intermittently (perhaps every month), to ensure neither
the fine details of performing them nor the continuing habit of keeping
them up to some degree was not being gradually lost.
